NOTE: Unless specifically noted below, each state allows an attorney to request a birth certificate for a client they are representing, provided the attorney includes picture ID and a letter certifying their representation of the client.

ALABAMA:

A person may obtain only his or her own birth certificate, except for parents who may obtain their own child's certificate. (Cannot be obtained by an attorney)

MAINE:

No application form is available.

To fulfill your request, the following information is needed:

  1. Name on the birth record.
  2. Date of birth.
  3. Name of city/town the birth occurred in.
  4. The names of the parents - please use the mother's maiden name.
  5. State your relationship to the name on the birth record being requested.
  6. Please include a daytime telephone number.
  7. Please make the check payable to: "Treasurer - State of Maine".

The fee for one certified copy is $60.00. The fee for additional certified copies of the same record is $25.00 each. Send self-addressed stamped envelope with your payment.

TEXAS:

Children of U.S. citizen parents board while abroad (i.e., while serving in the military) can order certified copies of the Consular Report of Birth Abroad re: their birth at:

NOTE: The application requires the person named on the birth certificate to sign the form before a notary public, as well as details re: the person's parents' birthplace and date of birth.Also, if the person has requested a certified copy before, the representative's letter must explain the 'disposition' (ie, lost, stolen) of that copy.
